If you have an analyzer connected, then make a simple boot disk of your
preferred flavour of OS that does nothing but reset the machine
continually, until it detects that the device is missing. The problem is
probably in the BIOS POST, and the initial enumeration of PCI devices
done by the BIOS.

Things that might change the frequency of occurrence: Any bios “quick
boot” options, powersaving options, etc etc. Any significant difference
between warm and cold boots?

Also to be considered is your device, whether it implements PCIe
sufficiently closely to the spec…

Time to dig the PCI specs out!
