I got the same problem while working with the BT USB driver from CSR. The device had been plugged/unplugged several times before the bugcheck occurred (not very fast, say once every 2-3s). I use the official release of Vista, BTW.
Callstack and bugcheck parameters would be helpful in determining what is going on
