I apologize for the lame OT question, but is there a support charge from
MS when having an altitude assigned
to a mini-filter? If so, how much?
Second, from looking at the start page for mini-filter altitude
assignment, company name and ect are required.
Can anyone tell me how this is/would be handled for an open source
driver where there is no company; or
is MS going to hassle people like how they do with the class-3 cert for x64?

No (well, we didn’t pay anything for ours)

Not sure; I think there is an email address for questions on that page,
although I haven’t looked in some time.
Altitudes are free, but your driver won’t load on x64 without a signature.
