I guess, the Cache Manager/IO Manager can hold on to a previously opened
file for quite some-time. How can a file-system re-director force NT IO
Manager to dispatch IRP_MJ_CLOSE right after IRP_MJ_CLEANUP?

By tearing the cache maps only. CcFlushCache, CcPurgeCacheSection.
Note that if you have a user mapping of the file, this will fail.
