windbg dies on host at breakpoint

debugging kernel driver. Everything works as expected except:
set breakpoint
execute, hit breakpoint, windbg on host dies (exits/closes down)
restart windbg, .reload, open src etc
shows target stopped at breakpoint
can watch variables, dis-assemble etc etc
but single step, run-to-cursor etc cause windbg to die
(can restart like above)

target NT 4.0 sp6, host NT4.0 sp5 windbg v5.00.2127.1
Tried windbg v5.00.2195.10, same result

Any clues appreciated

Use the latest debugger from http://www.microsoft.com/ddk/debugging
