I figured this out using SoftICE, not long after I sent my original mail,
actually! But thanks for the reply.

With respect to your problem with Create IRPs; I read on the list last week
(ironically) that they aren’t inherently asynchronous and so trying to wake
up a thread upon completion of a Create is fairly futile. this may not
always be the case, but it’s what I remember.
